1. Lifetime Partnership – If you becoming a lifetime member please complete this form along with your payment and send it to the state Treasurer.

2. local leadership – if there is any change in the leadership for your local women’s ministry please complete this form and send it to the State Coordinator.

3. CBO – Complete this form and send it along with your offering to National Office.

4. Honorary Gift – is there someone you would like to give a love gift in honor of.

5. National Mailing list form.

6. Blank rooster – please complete this form for anyone that is doing the basic membership and mail it along with the membership dues to the State Treasure.
